Community Rehabilitation
The Headway Dorset Rehabilitation & Brain Injury Service is part funded by health and social services. It provides community based specialist assessment and rehabilitation for people over the age of 17 years who have an acquired brain injury.
What we do

The team
The multi-disciplinary team consists of:

What we offer
The aim of rehabilitation is to reduce the impact of your difficulties on your lifestyle. This might involve helping you to adjust to what has happened, helping you to learn new skills to overcome difficulties you might be experiencing and helping you to improve your quality of life. This may involve:-

An initial assessment to clarify your goals and rehabilitation needs.

More detailed assessments by individual Clinicians.

A rehabilitation plan, tailored to your goals.
-
Providing you and your family with support, advice, and information relevant to your needs.

Linking you in to more appropriate Services or resources.

Reviewing your programme on a regular basis.
You can access the team either on both of the Headway Dorset sites, in Poole or Dorchester, visits by the team can be carried out in your own home and other group work and assessments may take place elsewhere.
Referrals
The service operates an open referral system. Referrals are open to all who are located in the geographical area served by Headway Dorset, from statutory agencies and voluntary bodies to family and self referrals.
Eligibility
You may be eligible for referral to the service if you:

Have an acquired brain injury.

Those referred must be assessed and show a need for the service provided, the predominant issue must be the difficulties engendered by an acquired brain injury.

Headway Dorset’s services cover all forms of acquired brain injury, however individuals with a brain injury acquired as an adult will usually take priority.

Are over the age of 17 years
