How do I refer myself or someone else

To make a referral we will need you to complete a short form with some basic personal details and background information. You can telephone us on 01202 606560 and we can complete this together or arrange for a form to be sent to you by post or email.

 

What happens next

Once we have received your referral, we will arrange an appointment for you to visit us for an assesment to ensure you receive the appropriate help.
